- 36.25 hour work week, full-time role. Hourly position paid bi-weekly.

- One week of paid vacation is immediately allocated upon hire. 2 personal days provided per year, prorated with employee start date. Starting vacation accrual of 3 weeks per year. Sick time accrued at 13 days per year with an unlimited accrual.
- Starting salary range, determined by formula accounting for years of experience: $49,000.00 (base rate) to $61,000.00 (10 years of experience performing this same job or job with primarily the same duties).

Support Navigator
A typical day in the life of a Support Navigator with Hamilton County Developmental Disabilities Services (HCDDS) includes managing and checking in with a revolving caseload of adults and children with developmental disabilities, serving as the primary point of contact for short-term needs. You will maintain and track regular contact with these individuals and families, providing them with resources and referrals to internal and community programs to meet their immediate needs. You will assist individuals with navigating and accessing various systems including but not limited to housing, schools, and social security systems. You will play an integral role in supporting residents in Hamilton County in receiving the services they need to thrive!
Job Duties (Summary):
- Serves as the primary point of contact for specific individuals served and families requiring less than full developmental disabilities services.
- Develops and maintains positive relationships with people served.
- Provides requested information and resources concerning eligibility throughout the community linkage process.
- Assures, with supervisory support, that all required documentation is completed and maintained in a timely and accurate manner consistent with all regulatory requirements.
- Maintains accurate documentation including reviewing case notes, logging events, and progress coordination of services; maintains confidentiality of individual files.
- Works a flexible schedule to provide necessary services.
- Performs additional duties as assigned.
Requirements:
- Bachelor's degree required; a degree in psychology, special education, human services, or related fields preferred.
- One (1) year experience working with individuals with DD in a vocational, residential, service & support administration, or equivalent preferred.
- Must be able to pass criminal background checks and various abuser registry checks.
